Upper West Side Medical Associates (UWSMA) is a single healthcare provider facility located in the Hamilton/Sugar Hill historic district in Manhattan at 464 West 145th Street, New York, NY 10031. UWSMA is also a member of the National Health Service Corps: NHSC.hrsa.gov. We serve all patients regardless of inability to pay. Discounts for essential services are offered based on family size and income.

 

The practice occupies the first and second floors of a brown stone house. Inside the waiting room the walls are decorated with historical cultural scenes and pictures of famous historical persons that contributed to Harlem’s unique history .The pictures on the walls are conversation pieces of reminiscence for most of our senior and young adult patient population.

 

We provide a high standard of care in a home like atmosphere. The waiting room is a cool comfortable place in the summer and a warm cozy place in the winter. It is convenient to most public forms of public transportation.

 

The bottom line is UWSMA is the place for getting good health care. We are a PCMH facility

That operates like a close knit family. We take into consideration patient factors such as culture, language, values and unique preference. We foster good and solid relationships with patients, family and team to achieve the best health care. We are becoming a PCMH because we believe in providing other services to help our patients achieve their long term goal for a healthy living. PMCH is a model to reinforce our fundaments of providing quality care.

Our Service

Dr. Myers and Dr sargeant are board certified physicians that specialize in internal medicine. They are committed to patient education and are recognized by IPRO annually for their outstanding participation and positive achievement in this area. One of UWSMA goal is to bring awareness to patients on the importance of preventative care. Diabetes education is held at the practice weekly by qualified diabetes educators.Hypertension counselling is provided by the medical staff. A weight loss program is available five days a week by appointment to patients diagnosed with obesity. Patients are advised and administered vaccinations and provided with prophylactic medicines, or medicines they need for travelling to countries outside the USA, e.g. Anti-malarial

Cynthia Myers, MD

Dr. Cynthia Myers has been practicing internal medicine for fifteen years. She has a great   relationship with her patients; most of them have been her patients from the start of the practice. A graduate of Temple University College of medicine, she participates in continuing education programs, training in new techniques and health programs that evaluate quality of care of her patients. She is the recipient of annual awards from IPRO and other health monitoring organization for outstanding services to her patients. In the summer Dr. Myers opens the back door of her office to her patients giving them access to her small garden where they can pick organically grown vegetables and herbs. This garden is tended by the staff and her. Dr. Myers has a signature smile of warmth that greets every patient that walks through her door.

 
Walt Sargeant, MD

Dr. Walt Sargeant has been practicing medicine for six years. A graduate of Temple university 2002, he joined UWSMA in 2008.He received the patients choice award (2008 – 2011) and the compassionate Doctor Recognition (2010-2011).He is affiliated with Lennox Hill hospital, St. Luke’s  Cornwall hospital ,St. Luke’s  Roosevelt hospital, and Harlem  Hospital Center.

UWSMA Sliding Fee Discount Program

Upper West Side Medical Associates will offer a Sliding Fee Discount to those who have no means, or limited means, to pay for their services. UWSMA will base program eligibility on a person’s ability to pay and will not discriminate on basis of age, gender, race, color, creed, ethnicity, religion, natural origin, marital status, sex, sexual orientation, gender identity or expression, disability, veteran or military status or any other basis prohibited by federal, state, or local law. The federal Poverty Guidelines, http://aspe.hhs.gov/poverty, are used in creating and annually updating the sliding fee schedule (SFS) to determine eligibility. 

UWSMA does not deny patients due to inability to pay and SFS availability
